Workers' compensation claims
A user ID is required to report online through our claims administrator, Gallagher Bassett. If you already have a user ID please click the Report Claim Online link above. If you do not have a user ID, please contact one of the managers listed below.
You may also report by phone using our nurse triage claim reporting line. This method is recommended to be done by the employee directly. Call: 833.956.8750.
